案例介绍：几年前求助者的儿子恋爱期间女友怀孕。求助者考虑到面子问题，坚持让其做了人工流产。婚后儿媳几年都没有怀孕。一天某位同事无意中说道：“你当年要是不让她做人流，不是早就当上奶奶了吗！”求助者当时没有在意，晚上想起同事的话，也觉得有道理，从此后“我当年要是不让她做人流，不是早就当上奶奶了吗！”这个想法经常出现，一出现就让求助者觉得后悔，为此很苦恼。求助者有时也劝自己：事已至此，想也没用，但始终控制不住，想摆脱但摆脱不了，内心非常苦恼。为此经常耽误家务事，影响了家庭生活，不愿意参加家族或同事、同学等聚会。求助者也知道有些事没有必要反复思考，但无法自拔。求助者焦虑不安，情绪烦躁，晚上难以入睡，白天没精神。自己迫切要求治疗。
心理咨询师观察了解到的情况：求助者性格较为内向，自我要求严格，做事认真细致，做什么事情都要做到最好。母子感情深。人际关系良好。
